Personal responsibility for measurable elements of input, throughput, or output that the individual feels emotionally and intellectually committed to.

Accountability is a responsibility that can be punished or rewarded because it is based on sufficient authorance and a clear reward system.

If responsibility is what the organization expects from someone then accountability is what she expects from herself.
